Eligibility

At least one must be met for the patient to be eligible for referral.

Patients who are being treated for the same condition at another Victorian public hospital

Children under 18 years of age

Date of birth • Reason for referral

Patient’s contact details including mobile • Duration of symptoms phone number • Relevant pathology and imaging reports

Referring GP details (Alfred or Sandringham Radiology

If an interpreter is required preferred to facilitate access to results)

Medicare number • Past medical history

Current medications

impacting on daily activities including impact on work, study, school or carer role, or

worsening or unresponsive to medical management and therefore further advice on, or a review of, the current management plan is required

Abnormal liver function tests

Atrial fibrillation

Chest pain

Chronic refractory diarrhoea

Headache

Inflammatory arthritis

Motor weakness or paraesthesia

Movement disorders and dystonia

New persistent or chronic pain related to COVID-19 infection

Palpitations

Stroke or transient ischaemic attack

Vertigo (neurology)

Red flags

Indicate urgent (Category 1) referral or emergency assessment.

Acute deterioration, life-threatening features or symptoms requiring urgent specialist input — contact the on-call registrar or refer to ED

Routine referral criteria

Determine Category 2 or 3 urgency.

Initial primary-care work-up complete (history, examination, baseline investigations per Alfred guideline)

Symptoms persist or progress despite primary-care management

Usually managed in primary care

Patients not able to actively participate in an assessment process or where the patient, or their carer, do not consent to being referred to a public hospital service
Patients who do not have a probable or confirmed diagnosis of Post COVID
Patients who want to receive services as a compensable patient should not be referred to a health service that only provides publicly funded services
Patients who are being treated for the same condition at another Victorian public hospital
Children under 18 years of age
