Bayside Referral Assist

Eligibility

At least one must be met for the patient to be eligible for referral.

Patients who are being treated for the same condition at another Victorian public hospital

Children under 18 years of age are not seen at The Alfred

Date of birth • Reason for referral

Patient’s contact details including mobile • Duration of symptoms phone number • Relevant pathology and imaging reports

Referring GP details • Past medical history

If an interpreter is required • Current medications

Medicare number

Progressively worsening oropharyngeal or throat dysphagia

Inability to swallow with drooling or pooling of saliva

Unresolved food bolus obstruction

Progressive dysphagia

Potentially life-threatening symptoms suggestive of acute severe lower gastrointestinal tract bleeding

Positive immunochemical faecal occult blood test (iFOBT)

Red flags

Indicate urgent (Category 1) referral or emergency assessment.

Progressively worsening oropharyngeal or throat dysphagia

Inability to swallow with drooling or pooling of saliva

Unresolved food bolus obstruction

Contraindications listed under General Indications statements

Fulminant Colitis

Documented acute diverticulitis

Potentially life-threatening symptoms suggestive of acute severe lower gastrointestinal tract bleeding

No FDR or SDR with CRC

1 FDR with CRC age ≥60

Routine referral criteria

Determine Category 2 or 3 urgency.

Initial primary-care work-up complete (history, examination, baseline investigations per Alfred guideline)

Symptoms persist or progress despite primary-care management

Usually managed in primary care

or is a condition not routinely seen at Alfred Health
or the patient lives outside the Bayside Health catchment area and requires a standard gastroscopy or colonoscopy (except where the patient is currently admitted under a state-wide service at The Alfred or has a documented history of previous endoscopy at The Alfred). Out of catchment referrals for complex endoscopic procedures continue to be accepted. Complex endoscopic procedures include:
Endoscopic Retrograde Cholangiopancreatography (ERCP)
Endoscopic Ultrasound (EUC)
Ballon Enteroscopy
Endoscopic Submucosal Dissection (ESD)
Endoscopic Mucosal Resection (EMR)
Peroral Endoscopic Myotomy (POEM)
Stenting
Capsule Endoscopy
Patients who are being treated for the same condition at another Victorian public hospital
Children under 18 years of age are not seen at The Alfred
