Stinging insect allergy

Insect sting reactions where venom desensitisation may be appropriate. Blood (specific IgE) or skin testing is performed at The Alfred.

Eligibility

At least one must be met for the patient to be eligible for referral.

Adult patient (18+) with history of reaction to insect sting

Candidate for insect venom desensitisation

Red flags

Indicate urgent (Category 1) referral or emergency assessment.

Acute anaphylaxis to a sting — refer to Emergency Department

Recent anaphylaxis without a management plan — contact Allergy registrar on 9076 2000

Routine referral criteria

Determine Category 2 or 3 urgency.

Documented systemic reaction following insect sting suitable for venom immunotherapy assessment

Large local or non-systemic reactions only

Usually managed in primary care

Document careful history of the sting episode (insect type if known, time interval to reaction, severity).
Document anaphylaxis if present and any adrenaline autoinjector already prescribed.
Children under 18 are not seen at Alfred Health — refer to a paediatric service.
